2022 KY HIGHSCHOOL COMBINE

In order to participate in the Kentucky Pro Football of Hall of Fame/Blanton Collier Sportsmanship Group football combine, I agree to the following statements: • I acknowledge that the symptoms of novel coronavirus (COVID-19) include but are not limited to: fever, fatigue, dry cough, difficulty breathing, and body aches. • I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days. • I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 30 days. • I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days. I agree to each statement above and hereby release the Kentucky Pro Football Hall of Fame/Blanton Collier Sportsmanship Group from any and all liability for exposure or harm that might come to me due to said exposure and COVID-19.

Intending to be legally bound hereby, the undersigned agrees and does hereby release from liability and to indemnify and hold harmless the KY Pro Football Hall of Fame/Blanton Collier Group, and any of its employees or agents representing or related to the Hall of Fame/Blanton Collier Group as regards to the High School Combine. This release is for any and all liability for personal injuries (including death) and property losses or damage occasioned by, or in connection with any activity or accommodations for this event. The undersigned further agrees to abide by all the rules and regulations promulgated by the KY Pro Football Hall of Fame/Blanton Collier Group and/or its affiliate groups and vendors throughout the High School Combine.
